Please note that all persons intending to attend the Congress must register, including invited Paper Presenters, Chairpersons and Communication Presenters. Please read through this website carefully before you complete your registration form.

Entitlements
Delegates Including Nurses/ Technicians & Other Health Professionals
The registration fee entitles all delegates to the following:

  • All Sessions
  • All official documentation including program booklet, proceedings/abstract book and list of participants
  • Welcome Reception
  • Opening Ceremony
  • Morning and Afternoon Teas
  • Exhibition Entry
  • Satchel

Trainee/Student
A student is defined as a holder of a student identification card from a recognised tertiary or secondary educational institution or international student card. Students must be studying full time to qualify for the discounted rate.
Student registrants are entitled to the following:

  • All Sessions
  • All official documentation including program booklet, proceedings/abstract book and list of participants
  • Welcome Reception
  • Opening Ceremony
  • Morning and Afternoon Teas
  • Exhibition Entry

Please Note: A copy of your student identification card is required when you submit your registration
form. Without this copy, the full registration fee will be charged.

Day Delegates
Day Registration entitles delegates to the following:

  • All Sessions on the nominated day
  • Program booklet
  • Morning and Afternoon Tea on nominated day
  • Exhibition Entry on nominated day

Payment of Fees
NOTE: Registrations will not be processed or confirmed until payment in full is received.
Within Australia: Payment may be made by cheque or credit card. Please note all transactions by credit card will appear on your statement as payment to Congress by THPL. Cheques should be made payable to APCHF 2008.
International: Payments from overseas must be made by credit card or bank cheque in Australian dollars only, drawn on an Australian Bank. Your name and full address should be typed or printed clearly on the bank cheque. You may not pay your fees by direct transfer.

Acknowledgements
Your registration and payment will be acknowledged via email or post with confirmation of your requirements according to your registration form. Your registration will not be processed or confirmed if payment does not accompany your form. You will not receive any further publications before the Congress. Your letter of acknowledgment will include any further advice necessary prior to your arrival at the Congress. At the congress you will receive the final program, the book of published papers/ Congress documentation and a list of delegates.

Cancellation and Refund Policy
Cancellations must be notified in writing to the Congress Managers.
Registration Fee’s:
Before Tuesday 30 October 2007 will incur an A$200.00 cancellation fee.
Before Friday 14 December 2007 will receive a 50% refund of fees paid.
After Friday 14 December 2007 there will be no refund.
Accommodation:
All deposits must be paid by Tuesday 18 December 2007
Deposit is non-refundable at Tuesday 18 December 2007
Social:
Payments are non-refundable at Friday 14 December 2007
Tours:
Payments are non-refundable at Friday 14 December 2007

Entrance To Congress
Each member of the Congress will receive a name badge on registration. The badge will be your official pass and must be clearly visible at all times to obtain entry to all sessions and to social functions. The APCHF Congress Managers expect appropriate behavior from attendees and reserves the right to revoke the badge and privileges of anyone behaving in an inappropriate manner.

Registration Desk
The Registration Desk will be located on Level 2 of the Melbourne Convention Centre at the information desk adjacent to the You Yangs Hall and will be operating as follows:

The Congress Managers will be on-site for enquiries at other times.

Insurance
Registration fees do not include insurance of any kind. It is strongly recommended that at the time you register for the Congress and book your travel you take out an insurance policy of your choice. The policy should include loss of fees/deposit through cancellation of your participation in the conference, or through cancellation of the Congress, loss of international/domestic air fares through cancellation for any reason, loss of tour
monies through cancellation for any reason including airline or related services strikes
within Australia, failure to utilise tours or pre booked arrangements due to airline delay,
Force Majeure or any other reason, medical expenses (including sickness and accident
cover), loss or damage to personal property, additional expenses and repatriation should travel arrangements have to be altered. The conference managers cannot take any responsibility for any participant failing to arrange their own insurance. This insurance is
to be purchased in your country of origin.

Australian Visa Application
All travelers to Australia, other than Australian and New Zealand citizens, are legally required to hold a valid visa to travel to Australia Visa’s are either issued electronically (ETA) with
nothing appearing in the passport or through application where a sticker is required in the passport (Non-ETA). It is advised to apply for your visa as soon as possible to ensure processing time. The minimum time frame stated for an Australian visa application is six (6) weeks.

Australian Visa Application / Supporting Documentation
Delegates applying for a non-ETA visa may present their paid confirmation letter to an Australian visa office to support their application. No other letters will be issued for this purpose. As a Congress delegate, you are not guaranteed to be granted a visa.
